The aetiological agent of leprosy is mycobacterium leprae.
Lepra cells. It occurs in large numbers in the lesions of lepromatous leprosy chiefly in masses within the lepra cells often grouped together like bundles of cigars or arranged in a palisade. The granulation tissue with a large aggregation of vacuolated mononuclear cells containing lepra bacilli is a characteristic feature of lepromatous to repeated trauma and may get secondary infected resulting to distortion and mutilation of extremities. These fibrohistiocytic cells usually have a basophilic or amphophilic cytoplasm that is laden with mycobacteria often arranged in stacks and aligning with the cells axis fig.
It is an intracellular pleomorphic acid fast pathogenic bacterium. The histiod variant of lepromatous leprosy is associated with cells that have a fusiform shape and may be arranged in a storiform pattern figs. Mycobacterium leprae is the aetiologic agent of leprosy affecting the skin and peripheral nerves.
In size and shape it closely resembles the tubercle bacillus. Lepromatous leprosy is more infectious than other types. Lepra cells lepra cells distinctive large mononuclear phagocytes macrophages with a foamlike cytoplasm and also poorly staining saclike structures resulting from degeneration of such cells observed characteristically in leprous inflammatory reactions.
Clinical manifestations are varied and reflect the hosts immune response to the bacteria. In size and shape it closely resembles mycobacterium tuberculosis. The infection is currently found in over 100 countries often located in high burden areas against a low burden background of cases.
Indistinct staining results from numerous fairly closely packed leprosy bacilli which are acid fast and resistant to staining by ordinary methods but may be vividly demonstrated by acid fast staining procedures. Distinctive large mononuclear phagocytes macrophages with a foamlike cytoplasm and also poorly staining saclike structures resulting from degeneration of such cells observed characteristically in leprous inflammatory reactions.
